What Causes Hot Flashes in Perimenopause

If you are waking up soaked at 3 a.m. or running hot in a room everyone else finds comfortable, something specific is happening in your hypothalamus. This is what it is.


What perimenopause is

Perimenopause is the transition leading up to menopause. It starts when estrogen levels begin fluctuating unpredictably, sometimes spiking above premenopausal levels before dropping, and ends one year after the last menstrual period, which is when menopause is formally complete (Harlow et al., 2012).

This transition typically begins in the mid-to-late forties, though it can start earlier, and lasts an average of four to eight years (Santoro et al., 2015). Estrogen does not decline in a straight line during this period. It fluctuates. That is why one cycle can feel like nothing has changed and the next can feel like a completely different body.


The symptoms

The range is wider than most people are told going in. Irregular periods are usually the first sign: cycles that get shorter, longer, heavier, or lighter with no predictable pattern. Beyond that:

About 75 to 80 percent of people going through the menopause transition in the US experience vasomotor symptoms (Freedman, 2014). They typically peak in the first two years after the final period but often begin years earlier, well within perimenopause.


How a hot flash happens

Your hypothalamus regulates body temperature through what researchers call the thermoneutral zone: the narrow range within which the body neither sweats nor shivers. In most people that zone is roughly 0.4°C wide (Freedman, 2014).

As estrogen levels fall and fluctuate during perimenopause, the thermoneutral zone narrows. Sometimes significantly. When it narrows, a small upward shift in core body temperature crosses the threshold. The hypothalamus reads it as overheating. Blood vessels in the skin dilate, and sweating starts.

That is a hot flash. It typically lasts one to five minutes. Heart rate rises briefly. You flush and feel a wave of heat from the inside out. The body is doing exactly what it was built to do. It has been told it is too warm.


Why night sweats are the same mechanism

Night sweats are hot flashes that happen during sleep. The biology is identical: the narrowed thermoneutral zone, a small temperature shift, the hypothalamic response.

What makes them feel different is timing. During sleep, core body temperature shifts naturally as part of normal sleep cycles, and blankets and a horizontal position give those shifts more room to cross the narrowed threshold. Waking repeatedly to kick off covers or change damp clothing then fragments the deepest stages of sleep, which affects mood, memory, and energy the following day. Those downstream effects often get attributed to aging or stress on their own.


What helps

The most effective treatments for vasomotor symptoms are hormonal. Hormone therapy, estrogen alone or estrogen with progesterone, works by stabilizing the estrogen fluctuations that narrow the thermoneutral zone. For most people who use it, it significantly reduces both the frequency and severity of hot flashes (The Menopause Society, 2022).
